Terms & Conditions
Emergency situations:
In the event of an emergency or non-emergency situation requiring medical treatment, I hereby grant permission for any and all medical attention to be administered to my child in the event of an accidental injury or illness. This permission includes, but is not limited to, the administration of first aid, the use of an ambulance, the administration of anesthesia and/or surgery, under the recommendation of qualified medical personnel. If I cannot be contacted and my child should require emergency hospital treatment, I authorise a member of the Uplift staff to sign on my behalf any written form of consent required by the hospital. However I understand that every effort will be made to contact me as soon as possible.
By agreeing to these terms and conditions I also realise that I am agreeing to not hold Uplift or any of its staff or volunteers responsible for any injuries that my child may sustain while taking part in the Uplift program. I understand that my child is participating in this program at his/her own risk.
